Grade 9 - 12 Registration Packet

1. Student’s Birth Certificate
   • certified document, not a hospital record
2. Health Records
   • latest physical and immunization records [per state regulations (105 CMR 220.000), students cannot start school without required health records]
3. Photo ID of Caregiver(s)/Legal Guardian(s) or valid passport
4. Proof of Residency
   • copy of lease, Section 8 agreement, mortgage statement, or property deed
5. Proof of Occupancy
   • most recent utility bill (ex. gas, electric, or internet)
6. If applicable, any court documents
   • caregivers’ custody agreement, legal guardianship, DCF Care, legal name change, etc. 
7. If applicable, 504 Accommodation Plan or Individual Education Plan (IEP)
