Registration Hours: Monday – Friday, 8:30 am – 2:00 pm
When you visit the Registration Office, there is ample parking at the rear of the
building by the Peter Pan Bus Terminal/Round House Plaza.
Our building is also close to the PVTA bus dropoff in front of the Academy of Music.
Registration Procedure

- Student’s Birth Certificate
- certified document, not a hospital record
- Health Records
- latest physical and immunization records [per state regulations (105 CMR 220.000), students cannot start school without required health records]
- Photo ID of Caregiver(s)/Legal Guardian(s) or valid passport
- Proof of Residency
- copy of lease, Section 8 agreement, mortgage statement, or property deed
- Proof of Occupancy
- most recent utility bill (ex. gas, electric, or internet)
- If applicable, any court documents
- caregivers’ custody agreement, legal guardianship, DCF Care, legal name change, etc.
- If applicable, 504 Accommodation Plan or Individual Education Plan (IEP)
If you are unable to acquire any of these items due to your circumstances, please contact the Registrar for additional support and assistance. Please see the cover page of your registration packet for a full list of approved documents for each requirement.
